Rate mood 1-10 and anxiety 0-10. Baby blues (mood swings, crying, overwhelm) typically resolve by week 2-3. If mood stays below 5 after week 3, screen for PPD.

Track Sleep Fragments
Total hours AND longest stretch. The goal isn't 8 hours โ€” it's trending upward. Most babies sleep 4+ hour stretches by week 6-8.

Note Feeding Details
Method, frequency, challenges. Fed is best. If breastfeeding hurts after week 2, see a lactation consultant โ€” pain is not normal after the initial adjustment.

Baby Blues: Mood swings, crying, anxiety hitting 60-80% of new moms in the first 2 weeks. Caused by hormone crash. Usually resolves by week 2-3. NOT the same as PPD.
PPD (Postpartum Depression): Persistent low mood, loss of interest, guilt, sleep issues BEYOND week 3. Affects ~15% of new moms. Treatable with therapy and/or medication. Screening recommended at 6-week visit.
Lochia: Postpartum bleeding. Rubra (heavy, red) โ†’ Serosa (pinkish) โ†’ Alba (white/yellow) โ†’ Stopped. Heavy bleeding returning after it lightened = call your OB.
Diastasis Recti: Abdominal separation from pregnancy. Check: lie on back, lift head, feel gap above navel. >2 finger widths = DR. PT-guided exercises help. Crunches make it worse.
